APPOINTMENT POLICY:

If you are unable to keep your appointment, please cancel your appointment as soon as possible in order to free that appointment slot for someone else.
 
Please call if you are running late for your appointment.  This will help us to make the necessary adjustments to the schedule.
 
Please bring your insurance card to every appointment.

Co-Pays are due at the time of the appointment.
We will then submit a claim to all primary and secondary insurances.  Once the claim is processed, we will bill you for any remaining balance.  

It is your responsibility to ensure that your insurance is active for the date of service and that your child is able to be seen by Dr. Flint.  Some Medicaid plans require that Dr. Flint be listed as the primary care physician in order for your child to be seen.  Please call your insurance company if you are unsure.  Failure to do so could result in you being responsible for the office visit charges.

Please refer to the Financial Policy form for details regarding statements and unpaid balances.  

We accept Cash, Checks, Visa, and MasterCard.  We do not utilize an outside collection agency but have chosen to work personally with you to satisfy any outstanding balance.

For Self-Pay patients, payments are due at the time of the visit, in order to receive the discounted rates.
